The national Identificatio Authority (NIA) introduced booking of appointments for NIA-GHANA CARD registration-related activities such as Information updates, stollen cards and new registration at their headoffice ONLY at the moment.
Information Update
This is a service for updating your personal
information at NIA. will
NB: Any update of your personal
information that leads to the reprint of your Ghana Card costs GHC 30.00.
All other personal
information updates are free.
Remember to come along with the following
information and documents:
1. For Change of
Name, you must the original copy of a Gazette Notification showing the
change of name.
2. For Correction
of Date of Birth, you must submit the original copies of documents proving
your correct Date of Birth e.g. passports, old Birth Certificate, School
Leaving Certificates and the document you used as a verification document when
you were registering for the Ghana Card. These documents do not include a
Gazette Notification issued after you registered for your Ghana Card.
3. For correction
of misspelt names, order of name or date of birth, you must submit any
Government-issued ID acquired before you registered for your Ghana Card.
NB: For the Information
Update service you must come along with your Ghana Card.
Card Replacement
This is a service for replacing lost, stolen,
missing, damaged or defaced Ghana Cards.
NB: This service attracts a fee of GHC 30.00
Remember to come along with the following
information and documents:
a. For a lost, stolen or missing
Ghana card, you must submit the original copy of a Police Report or Police
Extract reporting the loss or theft of your Ghana Card.
b. For a damaged or defaced Ghana
Card, you must submit the old damaged or defaced Ghana Card for a
replacement.
New Registration
This is a premium service for registering for a
Ghana Card
This service attracts a fee of GHC 250.00
Remember to come along with the following
mandatory document and information:
a. Your Birth
Certificate OR your valid Ghanaian passport;
b. The Ghana Post
Digital Address Code of your place of residence.
OR
a. One (1) relative of
yours who has registered and has been issued with a Ghana Card to vouch for you
in case you do not have a Birth Certificate or a valid Ghanaian passport; AND
b. The Ghana Post
Digital Address Code of your place of residence
NB: Your relative must
present his or her Ghana Card during the vouching process.
OR
a. Two (2) Ghanaians who
possess Ghana Cards and know you to be a Ghanaian to vouch for you in case you
do not have a Birth Certificate or a valid Ghanaian passport; AND
b. The Ghana Post
Digital Address Code of your place of residence
NB: The two (2) Ghanaians
must each present their Ghana Cards during the vouching process.
AND
Applicants with the
following Government-issued documents must bring them along for harmonization
of data - SSNIT ID, Driver’s License, NHIS Card, Tax Identification Number
(TIN) and Voter ID Card.
